Replacing G50 clutch

Pulling out of a toll booth today and shifting from 1st to 2nd, the rubber center of my G50 clutch let go and I ended up riding home in a flat bed tow truck . What is the part number for the correct spring centered disc? The car is an '89 and has 48K miles. I have only owned it for a couple of months and therefore do not know the way the car was driven or if this is the origional clutch. Should I replace the T/O/B and the PP at the same time? Anything else I should check? What about resurfacing the flywheel?
