Ended up with 1.5" adapters (not spacers) which pushed the wheel/tire out just where it needed to be.

This is a picture of a 315/30-18 next to a 345/35-15.
Spacer shown is 2" for rear with the 1" adapter for the front that was too small.

As a side note (that may be worth a data point) before I ran the 255/45-15s on the front I ran a set of 285/40-15s on front. They rubbed on the inner fender well just a bit when lock-to-lock. Took them off 'cause it was a real b!tch to parallel park.
