There were huge assumptions that the UN and the int'l community would kick in all the troops needed to secure Iraq. That absolutely never materialized in anywhere the numbers needed, or (foolishly) envisioned.

What's more, the entire upper echelon of the US military command structure turned over their staffs to new officers right after the war as Officers retired and were promoted. Also, the civilian administration rapidly changed over from Garner to Bremer, which also entailed several policy reversals. And the entire civilian side of things was always stark beyond belief. Rumsfeld insisted DoD be in charge of civil admin instead of State, who had always been in charge of these matters before. Well, DoD virtually ignored the realities of nation building.

This crap all has a massive effect on the ground. In the first several months after the invasion a window of relative calm was looted away and lost.
