Great timing on this thread as I have just recently failed a smog test for the same reason that bcoats describes. I wish I had my test papers here at work to give details.

Scott (aka 87 930) does a good job summarizing the possible causes. I have been searching this forum for info and had come to the same conclusions. My first step will be to do a thorough check for vacuum leaks, then I'll be checking the timing, check dizzy for proper advance functions, check all ignition components.

My idle is not consistent. Sometimes the car idles smoothly and at the proper 900 rpm. Other times it drops quite low and does not hold steady. She decided to act the latter way when I went through the smog test.
