Was this a fix back in the day?

I wasn't aware this would actually work. Really wore the hell out of all the chains and sprockets. Replacing everything and adding a set of Carrera oil feed tensioners.

Yes, it was somewhat common. They took a hydraulic tensioner and made it solid.

They were adjusted too tight. I run JB Racing mechanical tensioners, they are bulletproof. Typically used on race engines. They can be used on the street with no issues. Periodic checks while adjusting the valves is common practice. The inserts are a cheap fix instead of the money for Carrera pressure fed setup. Biggest mistake made when people buy the mechanical inserts is setting chains too tight causing premature wear on all timing components.
