Small noise from tranny in 4'th gear with load

End of last year i noticed that I had a small noise from my transmission when in 4'th gear and there was load on.
When idling or just running the noise was not there, only when accelerating or had some load on the tranny.
The noise is only in 4'th gear with load

I suspect that the four point ball bearing might be bad, and that this cause the 4'th gear to make noise du to more pressure on the gear

Any suggestions?

Is is a huge task to replace this bearing?
Does anyone have a link where this is done?
