Autopilot is supposed to shut off if you take your hands off the wheel or your head begins to nod off so my first question is why didn't these fail safes work and the second is if you have to be fully awake, both hands on the wheel and head upright and forward, what the hell good is autopilot??

Old ad then, their latest update actually increased the requirements for hands on wheel

"With the latest update (2018.21.9) that Tesla started to push last week, it looks like the ‘Hold Steering Wheel’ alert appears every 15 to 20 seconds that pressure is not detected on the steering wheel."

Also, the car should immediately start to slow down and then slow more and more the longer the hands aren't on the wheel until it stops...at least that's how it's SUPPOSED to happen, clearly it didn't happen in this case.
