I've personally seen rotors like this before.
I've also seen a kid that lost one of the two caliper bolts on his car use a wire coat hanger in place of the caliper bolt to drive the car to the parts store that I used to work in to get a new caliper bolt. I also had a guy bring back a set of brake pads complaining about them not working. When he pulled them out of the box, it took me a second to fathom what he'd done. He put the pads on with the pad material facing AWAY from the rotors towards the calipers. Both pads looked like someone had taken a round cookie cutter to the pad material. He got pretty sheepish when I pointed out that he'd put them on backwards.
