What does the Surgeon General do? You hear and see warnings from them, especially on cigarette packs, but what else do they do? Are they just the nation's top doctor? If they are a General, why is their rank that of a Vice Admiral and not a Lt. General? If they are in charge of the Health and Human Services, shouldn't they be a 4 star?

Well, a synonym for "General" is average.

Seriously though, there are various definitions.

One is a common definition, a high ranking officer, the other common one is a literal rank.

They are not the always same thing but can be.

So...not a General and not likely a Surgeon either...
