I am trying to imagine a scenario in which I might get out of my vehicle at a busy intersection. I suspect she thought she had put the truck in park before exiting and slamming the door shut. But why?

Probably auto locked when the vehicle reached 10 mph.

Like the many other 911 owners on this board, I own a car that I can reach in the window and start it while it is in gear. That would be really stupid, but it is quite possible. I don't do that because I am smart enough to safely operate a manual car with no ABS or traction control, or back up cameras.

No the operators of cars (can't call them drivers) rely on the car to do everything except decide where to go and how fast.

That looks like an operator failure, not an autonomous fail.

Did driver exit car, engine on, in reverse instead of park?

^^^ +1....and even then, most car doors won't open with the handle...you need to find and push the unlock button first. He did well.

Caster doesn't work when you're in reverse? Hmmm. Never thought about that before.

A long time ago I watched a story on 60 minutes about runaway cars.

Apparently the older column shift cars were more prone to this because the lever could easily slip from Park to Reverse.
- As these cars aged the gear lever indent became worn.
