I just had my 6 month checkup. I have a Humana Medicare Advantage plan that requires twice a year checkups. My next one will be in December.
(I'm told) due to insurance requirements - I'm asked to fill out patient forms each year. SIX forms - FIVE of which are double-sided. So in reality - I'm filling out ELEVEN pages of forms. I despise paper-whipping exercises and thought after I left corporate and my last business I was done with them. Now I'm right back in the frying pan, so to speak. So this time I filled out my name on each form and then drew an arrow downward with the words "Same As Last Time" on them...lol. I also verified with my provider that these same forms will be used next year, as far as they know. So I asked for a complete set upon leaving and this time I will fill them completely out - and make copies - leaving out the date. So all I will need to do is bring a copy with me each time and just fill in the date of my visit. My checkup came out great with zero issues except my bad cholesterol was a little high. Other than that I'm fit as a fiddle at 67! Carpe Diem and keeping up the fight against "the man"! ![]()

It's called redundancy. And it's more or less a waste of paper and time when a simple "update" will do.
