My New UT Rabbit Hole - The Flight Channel
 
I now, cannot look at anything else on UT. I am absolutely fascinated with how an accident happens, especially with aircraft. A friend of mine who owns his own older Cessna said that: Take any air crash, miss-hap or close call and 92%+ will be the pilots / copilots fault. The remaining is improper maintenance.

This channel series is very well done, with very thorough description and recreation of each aircraft, pilot experience, payload, etc.

Since part of my job is incident / accident investigations, I always find it takes at least 3 things together as the cause. So I look to the videos and they illustrate just that.

Great "Chain of events" study on the Gimli Glider incident.
Large jet, all engines quit.....and they landed safely??

They seem to do well for the most part. The Xb-70 - F104 crash they mislabeled some planes and this really was painful to watch after the title "Most Realistic..."

Assigning Pilot Error (PE) is not an exact science, 92% is way to high: The PE rate varies significantly between General Aviation, commercial aviation (and the numbers are further broken down into type/model/series of aircraft, passenger load, etc.) and military aviation.

Even in military aviation T/M/S is critical as well as each services operational environment that absolutely affects accident rates.

I would also add Manufacturing and Aircraft Design into your fault tree....The CH-53E and V-22 killed a lot of people because the designs and manufacturing techniques were pushing boundaries at the time.
