Ditto. Anytime I remove/swap of the wheels, I carry the torque wrench with me in case one comes loose or I missed one.

Me too… wheels with huge offsets or spacers.

It looks like a hub failure. If you look really close you can see the rotor still attached to the wheel just after it hits the car in the rear.

Some of those are funny, but most are scary. Is there no mandatory reporting for some of those issues to like the NHTSA? If the customer refuses work, how can a shop (in some of those instances) let them drive away in that car?

One of my buddies worked at a local family owned tire and repair shop. They had a car come in with broken and detached rusted out suspension in the rear. The owner refused to have anything done. They called 911 and had the cops look at it, and they impounded the car, and it was mostly likely sold as scrap. The owner sued the shop, and they had to spend money on an attorney to fight it. They did take lots of photos, and had the police report saying the car was just not safe to drive.
