Income from illegal activities, such as money from dealing illegal drugs, must be included in your income on Form 1040, line 21, or on Schedule C or Schedule C-EZ (Form 1040) if from your self-employment activity.

I knew a guy who did taxes for people who had...unusual...incomes.
People like hookers, clergy, etc. Apparently hookers do file taxes (at least the the morally upstanding ones).

Correct. They have gotten a lot of criminals on this. They could not pin them down on other crimes but did on this one. Al Capone was tripped up by tax evasion.

A respected accounting firm I worked for had an individual tax client who was a Marijuana farmer.
The line items on his profit and loss statement were: Income from Marijuana sales Expenses- Labor, Spring planting Labor, Fall harvest Of course these details weren't reported on the return. He filed Schedule C.
