has anyone ever lost a netflix DVD?

I lost one a long time ago, I think it was a Bond film. they prorate it depending on how new it is and some other crap. I think they charged me like $6.

From the Netflix website:

If the DVD was broken or lost while in your care, you can pay for it by going to the Queue and clicking on the 'Report Problem' link next to the disc, then choosing “I have lost or damaged a DVD and would like to pay for it.”. Replacement cost is $14 ($20 for Blu-ray).

I've lost a couple, but never been charged. I think it depends on how long you've been a member and what your track record is.

Did you tell them you lost it or that it was lost coming to you/going back to them?

The U.S. Postal Service is a reliable and fast way to ship DVDs, but we understand that a DVD may sometimes be lost during shipping through no fault of the customer.

So we replace lost movies at no charge. DVDs cannot be removed from the lost status until they are received by our warehouse. If a large number of lost DVD reports are filed for an account, the account will be placed under review and the customer will be notified via email.
