Wow- another brilliant investigation that determines in hindsight that something different should have been done. That is money well spent!

You need to read the article.

It reveals decisions made in the hours preceding the explosion.

It also touches on the fact that this type of work is very complex.
Some folks tend to over-simplify it in an attempt to understand it. It isn't just drilling a hole in the ground.
There are allot of people who do very difficult, very complicated, very dangerous work all so people can fill their gas tanks relatively cheaply.
Yet for some reason they are painted as the bad guys.
I can't figure that out. If I were a PR guy I could make a huge fortune just off the oil and gas industry. they sorely need that kind of help.
