How does stupid stuff like this happen? Engineering degrees from cereal boxes?

It is unlikely an engineers were involved. Most software people aren't engineers.

It also sounds like some of the errors might character recognition issues. If you use character recognition in a price list you are taking a chance.

Hopefully, access and door logs are being reviewed to find the source.
