Assuming no pot metal:
The edges would be bent upwards and torn if suddenly yanked off.

Look for evidence of nicks or scrapes on the bracket... could've been cut with any of a few of these that could get up between the plate and the bracket on one side, then bent back and forth until it broke on the other side: https://www.amazon.com/wire-cutters/b?node=553186

There's definitely an abrasion line where the top of the plate would have been.

As for the vibration causing a break - in less than 60 days? I only received my plate in the middle of September. Maybe over 30 months...
