Need a "Petition" App
 

Hey guys, I need some quick leads on info.

I need to collect signatures, names, addresses, tel no, emails for a neighborhood petition. Want people to be able to go to a website and fill it out, or fill it out on an iPhone. Will be max 4,000 persons signing.

Looking for a "petition" app. Anyone know of such?

As far as fields go, you probably need name and date, possibly address if it is neighborhood focused (4k in a neighborhood! wow!)

Use an online QR Code generator once you have the URL for the form to be filled out, print that on your fliers, etc.

Consider registering a domain name if you can come up with one that is topical, easy to remember, non ambiguous in spelling, etc to put up a splash page explaining it all. If you do that, shoot me a PM I'll host it for you.

Anyone know if Google Forms can include a signature? I tried building a form, it is perfect except I couldn't figure out how to get a signature block.
