Form data to database to export help..
 
Have a web project I need to complete. There is a local sports association in town that I belong to that holds player evaluations each year and they want to move this process online. In the past, the players would show up at an evaluation and the captains of all the teams would rank them by paper and then turn the grading sheets in and we would take DAYS converting everything over to a spreadsheet and assign everyone a total rank based on their eval level. I threw together a simple page of what the entry form needs to look like that the captains will fill out, but it has no functionality yet. >> Eval

We have a MySQL database available already on the server and the page can be built in HTML or PHP.

What I need to be able to make it do:

1. Have different login for every user so we can track who filled out the form and who didn't.
2. The output form will have to show the totals from each ranking that they received both in each division and sub division. (using the example site I posted above, we would need to see how many A1, A2, A3, AA1, AA2, etc., votes they received AND how many A, AA and AAA votes they received overall.
3. The output form will also have two added columns that the admin will fill in, (player name and protected team, if any, that are not on the form that the captains see when they enter the rankings as the whole point to this, is anonymity)
4. Must be radio buttons so they can only select one rank per player.
5. A button where they can clear selections and start over if needed.
6. A way to restrict them for entering more than one set of votes. (don't want them to be able to log on multiple times and vote)
7. A way to make them vote for every player before it allows them to submit the form.
( I will have to add a "N/A" column at the end of each row as we always have more numbered jerseys than players, but I want them to have to select any one of the rankings, OR N/A for every row)

The gist of this is, we want to the captains to go to a site, log in and enter their evaluation rank for each player on a form and then we want the submitted data to be tallied up and those grand totals be sent out onto one form whenever we decide to go and retrieve them. ( we need to give the captains a few days to enter the data and then we will pull it out at the deadline )

I'm sure there is something I'm forgetting...

But, as you can see, there are a couple of different things going on here that I need help building.

You could also accomplish this with Google Forms easily. Share the form entry link with certain individuals as well.

Google forms will do all the tabulating as well? (and hide it from everybody else?) I never even thought of using GFs! I will look into that now!
