I've seen these come in with rocker arms laying in the bottom of the head. Pull the valve cover and look.

I had that happen one time. It was extremely obvious that I had dropped a cylinder when it happened. This problem is not near as obvious. It’s an almost imperceptible miss. I’ll pull a valve cover just to be sure it’s not a bad cam lobe or a failed lifter.
