hand throttle ison early cis cars, like my '74. it basically mechanically connects to the actual throttle cable. physically pulling it up, you should actually see the accelerator pedal move down..it's like 'holding the gas' with your foot during engine warmup. later cars used an aux air regulator to do this via vacuum for warmup idle. cold, the car is supposed to be started with the hand throttle full up position..as it warms, you simply lower it until around 150 degrees..by then it's warm enough to not need the throttle to maintain idle. often times, the rubber part that connects the hand throttle to the throttle cable itself disintegrates and must be replaced..i've had to do it on mine once and once for a friend. there is a small screw that adjusts it. the plastic piece i'm describing, you must remove the handbrake assembly to get to..there is a small metal slider, whose position you can move along the throttle cable..this plastic piece butts up against it. pulling up on the throttle causes this plastic piece to 'push against' the slider, which in turn moves the throttle cable (one running from the pedal all the way to the engine). adjusted properly, once started, the engine will idle at approximately 3500, full up position and near operating temperature. i'm typing this at 3 am, so if it sounds confusing, let me know and i'll clarify more..but if your car's basic idle at start is extremely low, it's because this part is either broken, not adjusted correctly or you haven't been using it.

Do a search; there are half a dozen threads, some with figures.
