Wide Tire Width Applications
 
Anyone have info on recommended Min & Max tire sizes for the following widths?
11"
11.5"
12"

It's not as easy as it sounds.
Width vs sidewall height vs tire manufacturer all play into what size will fit. As a general rule of thumb for low profile tires, I use a tire width that is equal to the actual rim width.
For example an 11" wheel is actually 12" wide so I would target a 12"/305mm tire, 11.5" wheel = 315mm tire, 12" wheel = 335mm tire.
You MUST get exact measurements from the tire manufacturer that you will use. There are no standards and sizes will vary. Add an inch if using Pirellis, they are the worst offender.

I run 345's on my 11" wheels.

Are you running 345/35-15s on 11x15 wheels?
If so they are a perfect example of manufacturer variation.
Yokohama 345/35-15s are an actual 13.5" wide - too wide for a 11" wheel.
Pirelli 345/35-15s measure an actual 12.75" wide and will fit an 11" wheel.
I ran a set of Yoko 345s on 12x15 rims and they just fit.

I think kuhmos also run a little wide. 315/35/17 on an 11" rim bow out quite a bit.
