Rear Shocks loose

Hi, while my engine is out I thought I would do a quick check on all the suspension components. I found the bottom mounts of the rear shocks to be loose. Now I know where the clunking was coming from. I just put these in a few thousand miles ago. I'm wondering if I'm missing a special washer or anything. Starting from the inside I have the bolt head, a large flat washer, the shock and then a special locking nut. PET shows a washer that I don't have under the nut. Is this a lock washer, flat or other? Should I have replace this nut with new?

What year/model car are we talking about?

The SC/Carrera shocks use a bolt, no nut.

Maybe they weren't torqued to spec upon installation?

if i remember it was a flat washer just smaller than the rubber bushing.
