I am looking for a format from an automobile repair facility that I can use as a basis for a work performed document. With having people over to tension and replace belts I would like to be able to print off a document showing the work that was performed so they have record of it. Does anybody have a form which they can share with me.

You can always use a company like NEBS and get pre-made forms.
