just clean mating surfaces well and make sure to oil the threads on the studs before torqueing.

Make sure surface of the head is flat as well as that the valves have a good seal

Blown gaskets will have deformation (split, torn..etc) of the metal fire ring. Usually when the fiber material breaks down, the coolant will eventually work past the fire ring and split it in two.

I would not worry about the lifters unless you have coolant in your oil, and in which case i would be more concerned about your rod and main bearings. the blown out material will only cause coolant to touch the cylinder head (Which its doing already)..so no real problem there.

I think this is pretty normal considering a 20 year old head gasket, my '87 n/a had about 95k miles on the original when it went. My '88 951 had about 107k when i pulled the head and the gasket was on its way out also. Personally i think anyone on an original head gasket, its only a matter of time.
