I'm thinking belt holster. If it was a shoulder it would have extra loops for the shoulder straps to hook to.

Looks like a modified flap holser with the flap removed and a snap sewn on.

The "US" means nothing. There are a plethora of commercially available holsters available today with "US" embossed on them. Not all military holsters had it, and the majority of military belt holsters were of the flap type. This particular example was never official US issue in any service at any time that I'm aware of, nor is it a modified example of such. Those that were embossed "US" almost always had it on the flap. Sorry, but this is just a humble holster, and it may not even be all that old. Oh, and it looks too short for a full size 1911, and possibly too short for a Commander.
