UPDATE: Fixed! Was a Vacuum leak which has now been corrected and the car runs perfectly. Thanks for all the helpful suggestions.

Yeah i already replaced the throttle body shaft o-rings. They were original and had completely disintegrated.

Not sure about fuel pressure. Will have to pick up a fuel pressure gauge and check the fuel pressure as well. The car running better at high RPM might indicate that its not a fuel pressure issue but...

Haven't set the CO% since swapping out the MAF...

Haven't set the CO% since swapping out the MAF but will look into.

When the car is hot what would be worse- too lean or too rich mixture?

No vacuum leaks as that has all been checked out....

Swapped MAF and problem is now fixed. Smooth running and proper acceleration with no stumbles.

Still does not like hot...
