I've just use standard Zerks... (I think you can get a kit with a couple of dozen for $5 at harbour freight) I could get an old fashioned 35mm picture of my locations if you want and then snail mail it to you...
(no camera)....
so I cut my grease channels on the inside surface of the bushing material and then installed the zerk which stuck into the bushing about 1/4 inch... drilled a hole through the rest of the bushing material to allow the grease to push into the inside area... its the inside shaft that you want to spin, so the zerk sticking into the bushing just helps anchor the bushing so that it doesn't spin on the wrong surface anyways.....
the other install trick (covered extensively over and over... yes I know I'm preaching to the choir eric) is to make sure the bushing material does not stick out the side of the swing arm farther than the metal shaft shoulder....
