Is there a constant light clacking noise? On older / high mileage cars the stub axle wears below tolerance and the outer inner bearing race often cracks. If when you remove the wheel you find it is cracked you'll need to replace both the bearings and the stub axle. If you just replace the bearings it'll fail again pretty soon. Ask me how I know.

If it's just a low grumbling noise it's most likely just a bearing. The one on the other axle won't be far behind it either.

Those nuts are on pretty tight and have a cotter pin to lock up the nut - I'd be surprised if it's loose.
