Two thoughts.
First, it's possible the belt was tensioned too tightly. It should have about 1/2" of deflection.
Second, it sounds like the engine and alternator pulleys may not be in alignment. That could be caused by several things, including the alternator not being properly seated in the housing, the housing not being properly seated on the engine, or an improper spacer between the fan and the alternator. If the pulleys weren't properly aligned, it seems to me that it's more likely the belt would pop-off, as yours did.
