Sounds like the connecting bolt (A) (connects the shift rod to the trans linkage) has come loose, check it (make sure the "dimple" in the trans mounted shaft is visible in the hole on the shift rod) and then safety wire it back in place.
"B" could also have come loose...C&D are primarily for the side to side adjustment (centering) of the shifter. If when in neutral, the shifter does not sit in the center of the pattern, C&D are what you use to correct the centering.
Also, when you check the shifter, check the two mouning bolts on the shifter base, these could also be loose.
