Total failure of Centerforce Clutch

I replaced all my fuel lines on my "77 930 last October using replacements from Len@autosports.com. I needed to fill up the tank and treat the gas with Stabil for the coming winter. I was lucky to get back home because the clutch failed after shifting into second gear after leaving the station. Had I not made the last three lights in a row, it would have been the ride of shame. I finally got time remove the engine and tranny and this is what I found:

First some pictures of the patient.




The Centerforce kevilar pucks unbonded from the disk, everyone of them front & back.


The pressure plate was undamaged, but the flywheel has one small defect area. I think I can just stone that area and the flywheel will be OK. What do you think? For replacement, I plan to go with a KEP stage 1 with organic disk.

I would contact centerforce and let them know, looks defective to me. how thick are the pucks that came off?

I installed the Centerforce clutch back in the middle 90's when I rebuilt the engine. I can't remember who I got it from, can't find any paperwork yet. It probably had only 8k to 9K miles on it. It gave no warning of impending doom. It always had smooth engagement was able hold my engine's 450 HP.

The majority of the friction pucks very ground into fiber powder getting the car home, the remaining few were damaged, so not an accurate representation of thickness.

We had a clutch explode on our race car. I think it was over reved (2nd gear instead of 4th) one time too many. In my case, the clutch would not disengage.
