additionally there is this-

in 7th or 8th grade i had a teacher named richard horn, funny thing is he was bald and had a big red birth mark high on his exposed forehead. we 7th graders surmised that was the mark left when they broke his horn off...

another teacher's at the same school had last name german, i forget his 1st name. the rub here is that mr german taught spanish.

We had a USMC Major on Camp Pendleton whose last name was "Major", so he was "Major Major" just like in Catch-22!

I did some work at a company that had a guy that went through the legal process and changed his last name to 'Stillsmoking' after the company implemented a No Smoking policy at his facility.

One of the president's at a company I worked for was named Knut (first name), not a completely uncommon Norweigan name, unfortunately when we published the quarterly report for the stockholders and financial community the printer (supposedly unknowingly) transposed two letters and spelled it Kunt.

They were distributed by mail and shortly after the distribution, the error was recognized. Needless to say, they are very cherished quarterly statements these days.

I believe we changed printers after that as well as a few heads rolling in the HR department whose job it was to proof read the document for spelling and grammatical errors.
