My wife is 39, pregnant with her first child. She miscarried in Nov 2007 at 4 weeks. she just got a call from her doctor that she was positive for double Mutation for folic acid. on a fasting homocitine

Mutations in the MTHFR gene are not rare. They cause the blood to clot too easily. You wife may have a family history of early heart attacks, deep vein thrombosis, pulmonary embolus or other clotting issues. People with these mutations can have a very high rate of miscarriage. The good news is that treatment results in great outcomes. Depending on the severity, the treatment could be as simple as folate and a baby aspirin. If severe, it could involve daily lovenox injections as well as aspirin and folate. Some people even need lifelong anti coagulation.
The overall miscarriage rate for ALL 39 year old women is 50%. If you have had an ultrasound at 8-10 weeks that shows a good fetal heart beat, the risk of miscarriage drops to less than 10%. Consult a perinatologist and get a solid treatment plan. If you had a recent ultrasound that's OK, then you have good reason to be optimistic.
