His friend told him to put a big potato down his pants when Madoff complained about not being able to attract women.
So he did. Then he complained to his friend that women were not only not attracted to him, they were avoiding him in obvious disgust. That picture was taken after Bernie's friend helped him out again by telling him to "Put it down the FRONT of your pants, dumbass!"
