I'm guessing you're going at this from a product-liability standpoint. My two favorites of all time:

FlyMo (The hovercraft-lawnmower; I guess it was reasonably-foreseeable that some idiot would use it to trim his hedges.)

Space Shuttles (within reason)

IMS shaft in my 2001 Boxster. If it fails then bank a/c dwindles, my wife says I told u so and starts hitting me with something hard!

hitch-hiking, talking to strangers, lard, letting your dog off a leash, blood transfusions, eating uncooked food, investing in a company, going to college as a way of bettering your finances, booking a trip on the Titanic or Hindenburg, giving a kid a BB gun, firing someone for doing a crappy job, believing someone's professional opinion

Where do I start...Lets's see..not washing your hands every five minutes, or after playing outside. Playing outside in the sun. Riding your bike without a helmet, knee pads and/or body armor. Riding in the front seat of a car (with a metal dashboard) without a seat belt and/or child seat. Waiting for the school bus alongside a highway, with no adults present.
Well, you get the idea. Being an (semi)Old Fart, I could go on for hours. We have become a nation of wusses, thanks to litigation.

One of my many "issues" with California is ......Gas Cans. The Air Resources Board here has deemed that the basic gas can spills too much gas when in use. Soooo, they've come up with these "special" gas cans that manage to spill much more gas and the "special nozzles" fail after about 2 days of regular use, end result = Waaay more spills and sub standard gas cans.
