Do I need to break this down further?

I have a feeling I need to take the needle off, because I don't see any broken teeth.
The odometer works whenever it feels like it, which is once a year.

Looks like the cable drive might be damaged. The plastic ear that meets the metal drive. Can't see it that well. Have you looked at the cable it self?

At first glance, the plastic gear at top center of your first picture looks like it may be worn.

Also, my car recently was briefly flooded, and the speedometer was cutting in and out. The next day it was working fine and hasn't been an issue since. I never really found what caused it, but based on the water height, I would guess it was the cable pickup at the transmission and not the speedometer gauge itself. Since your gauge seems fine from your inspection, I would check your full connection to see if there's anything amiss.

On my car the metal gear in the third picture was spinning on the shaft with all the numbers on it. I took a punch and carefully tapped around the gear in three places to tighten the gear on the shaft. been working for a while now.

I checked that metal gear this morning and did notice that it moves in and out a little bit. I don't think that's the problem, though. Will crawl under the car and pull the cable and take a look and reseat it. It would be nice if it was as simple as the cable.
