Red face Help, Pressure plate Cracked

Hi everyone, looking for some help or to find out if anyone has had this problem.
My clutch started slipping and difficult to engage 1st and 2nd gear so i have just dropped the engine and box, split box from engine and found the pressure plate cracked, the clutch was renewed 3 years ago along with the updated cross shaft etc and has only covered 20k since, have i just been unlucky or is there somthing that i have missed that could have caused this ?. thanks in advance.
p.s. 89 carrera 3.2 G50 G/box

Is there only 20K (miles or kilometers) on the PP or was the previous one reused? There appears to be significant corrosion on it so I will conjecture that it failed from combined stress and corrosion - crack starts at a stress riser likely made "sharper" by a corrosion pit. Has the car been exposed to road chemicals or saltwater/salt air?

That is the rustiest pressure plate I have ever seen in my life!!! Hard to believe it was in an '89 model ... consider yourself lucky it didn't grenade into 1000 pieces!
