How'd This Happen

During the reinstallation of a fuel injector, I took a look down the inlet opening and noticed something strange. The strainer basket was twisted and bunched up inside the injector body. What would make this occur? The car showed no signs of this injector being a problem, but I had to poke around in there anyway. Managed to push the basket back down but still looks a little misshaped. Probably made the situation worse and maybe pierced the basket.

The other injectors looked fine as far as their respective baskets so i'll just replace the one injector and of course new o-rings for all six.

Anybody ever seen this before?
