The Reality about Cayenne S engine woes

So much has been whispered, said and written about the engine failure of the first gen V8, that it is difficult / impossible to separate the reality from the "myth" - does anyone on this forum have some factual information on this?
what is(are) the origin(s) of the issue?
is there a quantification of the occurence, i.e. percentage of the truck affected?
what are the tale-tell signs of an impending failure?
what is the fix and the costs thereof?

Hopefully, responses to this thread will provide all of us with useful knowledge.
