When a prescription med has been in use long enough and there is a good safety profile, the FDA may approve it for OTC use.

As for combination drugs, the process for that is very rigorous. Combining two active ingredients, even from previously approved meds usually requires new studies , new applications and extensive reviews. Such a process costs millions.
