For elevation - possibly wrong front sight height. This will depend on where the gas block is located (rifle length gas, mid-length, or carbine length). A lower front sight could fix this for you, but I'd want to try at a distance greater than 25 yards first...
For Windage - either your rails are off, the carry handle is off, or the front sight isn't top dead center and the groove for the alignment pin on the barrel is off a tad.
