Not always!
My bank uses an app that I suspect is quite secure, and the Square credit card app if secure as well. Both of them want the phone number to send a text message to and get I have to enter a code they text to me. And both send me an email that the app was recently used. So full two or three factor authentication. The bank's app will regularly ask me questions I filled out when I set up checking account at the bank. One question is about middle name of my father. His middle name was spelled differently than most, and she entered it wrong, (normal spelling) so I have to remember his middle name and then misspell it.
And the transactions were done on an iPhone, tied to me.
Of course, Experian, Apple, the IRS, the FBI, the CIA and lots of other big tech companies have been hacked so I am careful.
