They protect both ways. My coffee bean grinder sometimes blasts out about 800 volts when it gets switched to off
A few (expensive) things were dying that were on the same 240 volt cable from the breaker box. An electrician friend said to just put a surge protector between the grinder and the outlet, and everything has been fine since.
