Tile under the toilet, set the closet flange on top of the tile. If you don't wish to replace the closet flange (after thinking through the error of your decision) pay particular attention to the thickness of the wax ring that you use. You want to get a decent amount of compression on it, which may mean using a thicker one, or two of them.
Hard to say, since I don't know what type of flooring that you will be using.
Make sure you get the flooring dead flat in all directions at least beyond the area of the toilet base.
