Don't forget smaller portions. If it's as severe as you make it sound, he's already taking some sort of acid blocker/proton pump inhibitor, and he's had endoscopies that document significant esophageal changes, then a lap Nissen isn't a bad way to go. It isn't just a matter of physical discomfort, but there can be cancerous risks, as well.
