And that sharp decline in revenue is exactly what motivates (some) hospital admins to take advantage of the CARES act tiered reimbursement program for cases, hospitalizations, and deaths. It is not a "conspiracy" that incentivized medicine/care will always invite some level of impropriety, even if relatively small. And in many cases the motivation to do so is arguably well-intended. IOW, hospital admins faced with the difficult decision of laying off staff vs. "strong guidance" for CV upcoding may be more incline to choose the latter.
This is a well known phenomenon, and really came to light during the initial AIDS outbreak. The CDC director himself has acknowledged that it's a "thing".
