JFK started a big fitness challenge with "The President's Fitness Medal" or something similar which I remember participating in as a child in grade school in the late '60s. You had to do a certain number of push-ups and sit-ups to win the little medal, he understood the importance of fitness and health and instilling it early. No POTUS since has really kept up the tradition. I would argue that Trump set the worst example with his lard ass but it's never really been a leadership thing since Kennedy.
And as to your other failed analogy, (a serious pattern), doctors and nurses definitely consider that fat people and/or smokers have ruined their own health when they show up with heart disease.
