I had a very similar issue where i was going crazy, line by line assuming "something" was wrong or missing. Turns out it was the dumbest thing. They want the SS# and phone number formatted as XXXXXXXXX NOT XXX-XX-XXXX. I had "-" in both my SS# and phone number. As soon as I corrected that it went through!
